solid insights! I think the PHR element is quite underrated overall and will be making a comeback when properly tech enabled and embedded into retail phenotypes that patient-consumers actually get to drive with primary care physician co-pilots

I agree there is huge potential there but apparently this exceeds just great technology. Several attempts (ShareNotes for examples) have shown that people are often reluctant to contribute to their PHR directly. Patient reported outcomes (PROs) collected passively / with limited friction can serve as a bypass yet converging them with the PHR and translating them to actionable insights is a tough nut. From all it seems like a task for Google (> Amazon >> other) but I think per the complexity it will take more time.
